Your First Appointment

Allow for 30-60 minutes for your initial appointment. Please complete the Health History Form and bring it with you to your appointment.

Please dress in loose fitting clothing like athletic wear or sweatpants. If you're needing treatment for your knee or below please wear shorts. If you're needing treatment on your shoulders please wear a tank top. There is space to change clothing in the clinic if you want to change when you arrive.

Medical Arts Center, Suite 15, on 7th Street

The clinic is located in suite 15 of the Medical Arts Center on 7th Street between Bookcliff and Walnut. It is on the east side of 7th Street near St. Mary's Hospital across from Walker Discount Medical Supply and behind Tope Elementary School. Parking is available directly in front the building.

We offer affordable cash rates and time of service discounts for payment on the day of your appointment.

We will also bill your health insurance if you would like us to. To understand your coverage, please contact your insurer before your first appointment. Your insurance company can fax us your coverage information at 970-797-4842.
